After the finding lands
A system can have appeals and still have no failure condition. Nine stages decide whether a validated finding can change anything, and each absence has a name.

The error: Centrelink raised welfare debts by averaging a year of tax office income across fortnights.

Failure behavior
After a validated finding, this institution produces people who can only plead, one-off exceptions, forever, absorbed harm, recorded as satisfaction, paralysis or patchwork, acknowledgment theater, self-graded exams, re-earned errors, and the reversed loss returns with nothing attached. 8 stages unanswered out of nine.
Filling a stage is not the same as satisfying it. “Officials retain discretion to ignore the finding” is an answer to the consequence question, and it is a failure behavior.
